Get the Care You Need To Live At Home
With AMERIGROUP Community Care's Community Connections Plan, we offer managed LTC designed for Medicaid-eligible individuals with chronic or disabling conditions that qualify them for nursing home level of care. We provide you or your loved one with the services that you need, many of them right in your home, so you can live in the community for as long as you are able. AMERIGROUP Community Care's Community Connections gives you one source to access your health care needs.

Your Health Care Team
When you join AMERIGROUP Community Connections, you will be provided with a Personal Health Advisor (PHA), a registered nurse who is experienced in coordinating community-based services for those with chronic illness. Your PHA will make sure that your health and social welfare are the number one priority. Your PHA will develop a personalized plan of care with you and will work with your physicians to make sure that you get the appropriate care for your conditions.

If you have specific language needs, AMERIGROUP Community Connections will make oral interpretation services available for 150 non-English languages free of charge, upon request, to all enrollees and potential applicants through Language Line®. Written information is available in Chinese, Russian and Spanish languages.

You also have access to the AMERIGROUP Community Connections Nurse Advice Hotline 24 hours a day, 7 days a week. If you cannot reach your physician or assigned Personal Health Advisor and you have any medical questions, you can call AMERIGROUP Community Connections and speak to a nurse. The Nurse Advice Hotline can give you advice about whether you should go to the emergency room, how to deal with a personal crisis at home, or provide instructions on how to take your medications in accordance with your prescription when your Personal Health Advisor is not available.

Covered Services

The above services are included in your AMERIGROUP Community Connections Managed Long Term Care benefits package. Services not covered by us (physician care, hospital visits or emergency care) can be coordinated by your Personal Health Advisor.

Who is Eligible?
Membership in AMERIGROUP Community Connections is strictly voluntary. To be eligible, you must:

• Be age 18 or older
• Live in the Bronx, Brooklyn, Manhattan, Queens or Staten Island
• Be eligible for Medicaid (according to New York Human Resources Administration- HRA criteria)
• Be able to live at home without jeopardy to health or safety
• Be eligible for nursing home care (according to the New York State standard assessment guidelines)
• Require at least one of the following long term care services and care management provided by AMERIGROUP Community Connections for 120 days or more:
• Nursing services in the home
• Therapies in the home
• Home health aide services
• Personal care services in the home
• Adult day health care
• Social day care if used as a substitute for in-home personal care services

An enrollment nurse will assess your needs and help you determine if AMERIGROUP Community Connections is the best option for you or your loved one.

How Your Services Are Paid
AMERIGROUP Community Connections is paid by New York Medicaid to provide the long term care benefits included in your membership. If you have Medicare, your benefits do not change when you become a member of AMERIGROUP Community Connections.
