About our Plan
We offer assistance to seniors so they are able to remain at home or live in an assisted living facility. The program provides help with day-to-day tasks such as dressing, housekeeping, bathing, meals and walking. AMERIGROUP can give seniors the help they need to live safely in their homes and avoid residence in a nursing home.

Eligibility
To qualify, you must:

• Be 65 years of age or older
• Be enrolled in Medicare Parts A and B
• Live in the service area
• Meet clinical eligibility requirements (for example, need help with daily living activities like bathing, dressing, eating, toileting, transferring or walking/mobility or have a chronic condition requiring nursing services), and
• Meet financial eligibility requirements (Medicaid eligible up to the Institutional Care Program Level)

You will receive

• Services at no cost, except for assisted living or nursing home facility services (where there is a cost to the member)
• Many benefits beyond Medicare coverage
• Services tailored to your individual needs
• A personal care manager to help you arrange services
• No need to change doctors
• Health Promotion

AMERIGROUP is not a Medicare plan. Our services are designed to work with your other health care benefits. You will receive AMERIGROUP benefits in addition to your Medicare coverage.
